Lower Blepharoplasty is also known as Posterior blepharoplasty. The primary aim of lower blepharoplasty is to remove the stuffed fat around the eyes and hence to enhance the facial looks.. This can also be undertaken to move the fat to hollow area.
Factors to be taken care before performing surgery:
- Preexisting Dry Eyes: May become exacerbated by disrupting the natural tear film.
- Looseness of the lower Lid Marign (edge):which predisposes to lower lid malposition.
- Prominence of the eye in relation to malar (cheek) complex.
Lower Blepharoplasty Process- The anesthesia used in the surgery called Transconjunctival blepharoplasty is local. Patients face is prepared with Betadine paint. After drying the skin, the lower eyelid is pulled away from eyeball. During this time a plastic plate is put over the eyeball so that this is well protected. Now initial incision is created beneath the eyelashes, through the skin. In the middle of the eyelid a structural layer called orbital septum is opened to expose the fat pockets .The weakened orbital septum is armored with structures, like in hernia repair. As Transconjunctival blepharoplasty is performed by removing lower eyelid fat through an incision on the back of the eyelid, so the need for an external incision is eliminated. Which further cause most natural outcome means almost no scars and reduces the possibility of hollowing. Other advantages include less bruising and swelling, less post-operative restrictions on activity, no structure to remove and a faster recovery.
Postoperative care:
- Ice compresses are recommended for the first 24-48 hours to control the swelling.
- Patient should avoid strain and stay under observational area for at least 1-2 hours.
- Ocular lubricants with artificial tears and nighttime lubricants are recommended, its a must for the patients possessing history of dry eyes or if lagophthalmus is present.
In case of unusual pain in eyes or visual disturbance merit careful of the surgeon. Clear lines of communication between the patient and surgeon should exist, particularly in first few days after surgery.
